PRIVACY POLICY

Your privacy on the Internet is important to us. As a requirement for registration, we obtain certain types of information about communicationinmedicine.org users. The following describes this information, what we do with it, and how to correct or change the information if you should desire to do so.

Scope of this Privacy Policy
This privacy policy applies only to the web site at www.speaksooner.org (the “Site”). The Site is an interactive health, science and technology website owned and operated by Center for Communication in Medicine, a division of The Institute of Medical Humanism Inc. (“CCM”). As used in this Privacy Policy, the terms “we,” “us” and “our” refer to CCM. The Site is available to you (“User“ or “You“) in connection with a limited license as set forth in the Terms of Use [hyperlink].

INFORMATION COLLECTED BY CCM

CCM collects two types of information from users: personal data (such as names and e-mail addresses) (“Personal Data”); and aggregated information (such as information about traffic patterns on the Site, for example, how many users log into the Site on a regular basis daily, weekly etc.).

Personal Data
Personal Data is collected through the registration process and use of the Difficult Conversations Workbook. Personal Data may include identifying information, such as name and email address, that will be used to generate your user profile. Your personal data will not be sold, given away or used for marketing purposes. We reserve the right to share your Personal Data with third parties if we determine, in our sole discretion, that such action is necessaryto comply with legal requirements or with legal process served upon us.

Aggregated Information
Aggregated information is collected through various methods: It may come from survey information and/or Site registrations, and/or IP addresses. Aggregated information may include but not be limited to age, gender, race, and other information. Some of these questionnaires or surveys may also ask you to provide health-related information. We collect this information to help us personalize your experience on our Site by tailoring our content to your specific situation and concerns, as well as for us to better understand who uses our Site and to improve our Site and online tools accordingly.

An IP address is a number that is automatically assigned to your computer whenever you’re surfing the web. Web servers, the computers that “serve up” web pages, automatically identify this IP address, and when you request a page from the Site, our servers log your IP address. CCM collects IP addresses for the purposes of system administration and to provide aggregate data for customization. CCM does not link IP addresses to any personally identifiable information. Although your session on the Site will be logged through your username, this process will serve to track your session information, and collect aggregated information. If this aggregated information is published or transferred to any other person, it will be in an anonymous format. Aggregated Information may also include user IP addresses, browser types, domain names, and other anonymous statistical data involving the use of our Site.

Use of Data
In connection with the Site, CCM may collect data related to Site users, including but not limited to the names, email addresses, demographic information, Toolkit responses, stories or other material submitted by users (collectively, the “Site Data”). All Site Data shall be the sole property of CCM. Without limiting the foregoing, CCM may distribute, sell, license and otherwise use Site Data in aggregate form only (so that no user is personally identifiable) for services provided by CCM to its partners, academic and other publications, research projects and other purposes

CCM agrees that it will not disclose to third parties the following information: User names; User email addresses; workbook answers. Notwithstanding the foregoing, CCM may disclose any such information in accordance with a valid judicial or other governmental order, provided that CCM shall have given you reasonable notice and opportunity to object prior to such disclosure.
As time passes, CCM may be sold along with its assets, or other transactions may occur in which user data (including but not limited to Personal Data) is one of the business assets transferred. In such a case, the user data which we have gathered may be one of the business assets we transfer.

Use of Cookies
The Site does use cookies in a limited manner. A cookie is a small data file that most major websites write to your hard drive for record-keeping purposes when you visit them. Cookies allow website managers to measure activity on the site and to improve your user experience, for example, by remembering your passwords and viewing preferences, thus allowing you to visit various registered access parts of the site without re-registering.

These cookies are not created or used to collect any information (aggregate or personal) about you at any time. These are the only cookies that exist in this website. You may occasionally get cookies from other sites, which is standard in the online world. CCM does not control these cookies, and these cookies are not subject to CCM privacy policies.

Public Forums
We feature public forums such as message boards, bulletin boards or similar activities where you and other users of our Site can communicate with one another. THIS PRIVACY POLICY DOES NOT PROTECT YOU WHEN YOU USE OUR PUBLIC FORUMS OR PROVIDE INFORMATION (INCLUDING PERSONALLY IDENTIFIABLE INFORMATION) ABOUT YOURSELF. You should be aware that any information shared in a public forum such as a message board, bulletin board or other forum is public information and may be seen by third parties that do not adhere to our Privacy Policy. You should think carefully before disclosing any information in any public forum, including the forums on our Site.

Security
The Site has security measures in place to protect the loss, misuse and alteration of the information under our control. However, data transmission over the Internet or wireless networks cannot be guaranteed to be completely secure. Although we are committed to safeguarding your privacy and security, we cannot ensure or warrant the security of any information you transmit to us, and you do so at your own risk.

The CCM Site may contain links to other sites, and those sites may not follow the same privacy policies as CCM. For instance, if you click on a hyperlink on the CCM site, this may take you away from www.speaksooner.org and to an entirely different site. This can include links from advertisers, content providers, and others who may use the CCM logo, brand, and or style as a result of a partnership arrangement. These sites may send their own cookies to you, and may collect and use data in ways that CCM would not. Please note that we are not responsible for the privacy practices of third party websites. We encourage you to be aware of this when you leave our Site and to read the privacy policies of every website that collects personally identifiable information from you. The Privacy Policy applies only to this Site.
Children’s Privacy
We are committed to protecting the privacy of children. Our Site and online interactive features are not designed for or directed to children under the age of 18. Our registration process is intended to ensure that only users who are over the age of 18 gain access to the Site. Children under the age of 13 are not permitted to use this Site. Changes to this Policy
We retain the right to make changes, in our sole discretion, to this privacy policy. If we decide to make an important change to our privacy policy, we will post a notification and link on our homepage so that you can review the updated policy. If at any point we wish to use your personal information in a manner different from that stated at the time it was collected, we will notify you by means of an email and give you the opportunity to indicate whether you consent to the proposed use.
